Add a question to the form

Ask parents an extra question when they apply to a grade.

Use this to ask things your school needs to know, like the child's previous school.

Registration form settings, Vacancies tab: a table of grades with their campus, number of seats and an Open status.

Steps

Open Admissions → Registration form settings.
Select the Grade setup tab and open the grade.
Select Questions.
Select the Add button.
Type the question and pick an answer type.
Add choices if the question is a pick-one or pick-several type.
Tick Required if parents must answer.
Save the question.
